Please read through the following before filling out the online entry:

1)  Please include as much information as you have.  An email address is required for each player.  If you don't know your amateur's name or email, just enter TBD (email example: tbd1@tbd.com). Please DO NOT enter any info for someone not attending.

2)  We will be sending each player a questionnaire to gather their information and later an individual invoice for payment (unless you specify that you are paying the full team's final invoice).  Payment is due by December 1st though feel free to pay sooner.

3) You will be charged a $100 deposit for your entry.
